ATTENDANCE: 180 SCHOOL DAYS PER YEAR REQUIREMENT: MONTHLY attendance records for the program with each school day consisting of at least 4.5 school hours for an equivalent of 180 school days within a 12-month period as required under the Georgia Law (O.C.G.A. Section 20-2-690) is required to be kept by the Parent and submitted to: TRINITY CENTER (only) for accreditation of courses on Trinity Center Monthly Attendance/Time Sheet Form

NOTE: All days of Attendance of the Student (including days of attendance in tutorial classes, online ….) shall be kept on the monthly attendance/time sheet form. Put an X in each day that represents a day of School.

TIME: A minimum of 120 hours per subject per year is required for each full credit, and a minimum of 60 hours per subject per semester is required for each half credit.

Record the time Student spends in each subject whether in independent study, online study, or tutorial class on monthly attn/time sheet.

(IF taking a tutorial class, show time spent in class and in independent study).

IF TAKING A DUAL ENROLLMENT OR GAVS ONLINE COURSE-Just list the course on each monthly attendance sheet; but time does not have to be kept.

Student should be able to keep time journal, but it must be verified by the Parent/Primary Educator. You List the subjects, put time increments in the days of the month. Time should be totaled Days this Month by hours/minutes and Time Year to Date (school year Fall to Spring) totaled by hours or minutes.

***This Attendance/Time sheet is to be turned in monthly to TRINITY CENTER (only).

IT IS REQUIRED THAT EACH STUDENT/PARENT COMPLETE THIS FORM MONTHLY TO RECEIVE CREDIT OF EACH SUBJECT.

This is required by the Georgia Accrediting Commission so that Trinity Center can show that each CREDIT that is given meets the requirement.
